MISSION STATEMENT
To provide and promote accessible comprehensive individual and community health care to the people of Moloka’i with respect and aloha.
VISION STATEMENT
That all people have optimal physical, mental and spiritual health.
HISTORY
The Moloka’i Community Health Center is owned and operated by the Moloka’i Ohana Health Care, Inc., a private nonprofit corporation established in 2003.
The corporation has a community-based membership that elects representatives to the Board of Directors. The basic philosophy of the organization is to provide quality health care regardless of the ability to pay and to deliver services in a nondiscriminatory manner.
The Health Center has been in operation since March of 2004. Operational funding in provided from the federal and state governments, third-party reimbursers and patient revenues.
STATEMENT OF CONFIDENTIALITY
As a volunteer of the Moloka’i Community Health Center, please be aware that patient and employment records are confidential, and may not be disclosed to anyone who is not authorized to receive it. You may not seek access to confidential information except when required to perform duties at the Health Center. This information includes addresses and phone numbers of anyone including family, friends and acquaintances.
Strict laws and guidelines govern disclosure of confidential patient and employee information unless the person receiving the information has a legitimate business reason to know. In all cases, a written or signed consent is required. You may not release ANY of this information unless instructed by your supervisor.
Any breach of confidentiality and/or unauthorized release of privileged information will result in corrective action, including termination of volunteer service, and possibly civil penalty.
